My DNP project focuses on the implementation of Diabetes Self-management Education (DSME) through shared medical appointments to reduce the high rate of uncontrolled type 2 diabetes mellitus.
Week 1: participants will be recruited through a convenience sample of current medical practice patients with uncontrolled type 2 diabetes who meet the inclusion criteria. Participants diagnosed with T2DM adult will be selected using data from the DNP student electronic medical record of eligible patients. Participants will be asked to sign an informed consent, be educated on the intervention, and the purpose of the project.
Week 2-9: The day before the appointment the DNP student will review the medical records and send reminders to the patients for labs, test procedures as well as referrals. The patient will then place be scheduled for a 90-minute Shared medical appointment and a discussion on diabetes will occur. The study participants will be called
