Adverse Event or Near-Miss Analysis

Despite the technological advancements aimed at improving patient safety and quality of care delivery, adverse events and/or near misses continue to be reported in healthcare practice. A near miss, as described by Patient Safety Network [PSNet] (2019), is an incidence with the potential to cause harm but was timely identified and the mistake prevented. According to Ahsani-Estahbanati et al. (2022), the decision to use medical procedures and medications for disease treatment or prevention involves a thorough analysis and balancing of the potential risks and benefits of the treatment as well as the potential adverse effects. Adverse effects may occur due to medication errors, poor hand hygiene practices, and adverse drug reactions that cause undesired unintentional harm to patients (including physical, emotional, psychological, and even financial harm) as a result of medical procedures or medications, rather than the underlying disease (Alhassan et al., 2019). The cause may be a substance in a medication, human error, and device/ equipment failure, among others. According to Er and Altinbaş (2023), more than 250,000 patients in the United States experience AEs every year, out of which more than 100,000 succumb to their effects. This paper reviews an example of an AE experienced in practice, including its implications to stakeholders, quality improvement (QI) related to the event to lower risks and enhance patient safety, and a QI initiative to prevent such events in the future.

An Adverse Event Experienced

An example of an AE experienced is that of a 42-year-old male who presented with rectal bleeding. The physician, following assessment conducted a limited sigmoidoscopy only.
