 My DNP project focuses on the implementation of Diabetes Self-management Education (DSME) through shared medical appointments to reduce the high rate of uncontrolled type 2 diabetes mellitus.

Week 1: participants will be recruited through a convenience sample of current medical practice patients with uncontrolled type 2 diabetes who meet the inclusion criteria. Participants diagnosed with T2DM adult will be selected using data from the DNP student electronic medical record of eligible patients.   Participants will be asked eligible patients.   Participants will be asked to sign an informed consent, be educated on the intervention, and the purpose of the project.
