Interview Summary

I interviewed Stella, who is one of the heart failure specialist nurses from XYZ Hospital.   Stella is tasked with providing care to patients with HF including education and follow-up. The XYZ  facility is a 300-bed facility that provides care to patients across the lifespan.  During the interview, Stella identified several issues among the key issues being 30-day readmission in patients with heart failure.  She stated that even though the facility has been adopting interventions such as medication reconciliation, optimizing discharge planning, and patient education, 30-day readmission has remained relatively higher.  The culture of XYZ is based on patient-centered care, teamwork, and quality improvement.  Stella has been involved in various interdisciplinary collaborations such as those aimed at enhancing outcomes in patients with HF and improving the discharge protocol.

Issue Identification
