NURS-FPX4010 Assessment 1. For this assessment, you will create a 5-10 minute video reflection that addresses either an interprofessional collaboration you experienced or a collaboration case study scenario that you imagine you experienced.

Using Kaltura, record a video (5–10 minutes) where you reflect on an interprofessional collaboration experience, proposing recommendations for how to improve interprofessional collaboration that can be shared with leadership and colleagues. Support these recommendations with references to the literature.

            The interdisciplinary collaboration experience I have recently had involved the care of an elderly patient, a 71-year-old Caucasian male with a history of heart disease and diabetes type 2 admitted to the emergency unit for chest pain and shortness of breath. The interdisciplinary team included a nurse (myself) who provided direct care, a cardiologist who offered specialized cardiology care, an endocrinologist who specialized in diabetes management care, a pharmacist who oversaw medication management, a respiratory therapist who helped in managing the respiratory symptoms, a physical therapist who offered assistance in rehabilitation and mobility, and a social worker who helped address the patient’s psychosocial and discharge planning needs. During assessment and diagnosis, the nurse (myself) conducted the initial assessment, including taking vital signs and reporting the patient’s condition to the cardiologist who then assessed his heart condition, diagnosed a myocardial infarction, and prescribed the appropriate treatment plan. I also offered ongoing education to the patient and his family about his condition and treatment plans throughout his care and during discharge planning. The endocrinologist evaluated his diabetes management plan and recommended adjustments to the treatment plan. The pharmacists ensured the safe administration of the patient’s medication, including reconciliation and consideration for potential interactions. The physical therapist was involved in developing a mobility and rehabilitation plan for the patient during discharge to help him regain strength and function.
