Leading Change and Creating the Evidence Base . Leading and managing change is an important part of successful project execution and implementation. The nurse leader and project manager should be intimately familiar with a particular change theory or model that serves as the framework for leading the change represented by a project.
Spend time researching and investigating some evidence-based change models such as Kotter, Lewin, Lippitt, & Rogers (aka – Diffusion of Innovations). Identify your “go-to” change leadership framework and discuss how you will use that framework to guide organizational change.
Application: For change models with a series of steps or phases, identify which phase/step you are at now with your project and how the change model guides you in moving to the next phase/step. Link to week 4 Assignment

My project focuses on the implementation of a comprehensive fall program comprising of safety audit to identify gaps, staff training, and technology such as bed alarms to reduce the incidence of falls in rehabilitation and nursing care centers. The model of change that I will use is Kotter’s 8-Step Change Management Model. This model was designed to help the leaders in the successful implementation of organizational change. These steps include:
- Create urgency: The first is creating urgency about the need for change (Miles et al., 2023). I did this by communicating the high rate of patient falls in the facility. This encouraged the team involved to accept the change.
- Build A Guiding Coalition: This involves building a team to steer the change and ensure that the team has the necessary skills. I completed this step by selecting team members to be involved in the fall prevention program such as nurses and physical therapists among others.
- Form A Strategic Vision: This involves clarifying how the future will be different when compared with the past change (Miles et al., 2023). I did this by developing a clear vision for the project on how the project will reduce patient falls by 20%. Initiatives such as safety audits and staff training were initiated.
